Vascularized Lymph Node Transfer
Vascularized lymph node transfer is a surgical treatment method for chronic lymphatic edema of the upper extremities, including the hand. This procedure involves creating new pathways for lymph drainage from the affected area using functioning transplanted lymph nodes.
Before the procedure, you will meet with the attending physician. The appointment will include a medical history review, an examination, and additional lymphatic system tests. These tests are necessary to determine if surgery is indicated. The selection of compression stockings according to individual measurements will be discussed before the surgery. If surgery is recommended, the plastic surgeon will discuss options for donor flap sites (supraclavicular, inguinal, axillary, etc.). As much as possible, swelling in the area of intervention should be eliminated before the procedure with nonsurgical treatment under the supervision of a lymphologist, specializing in complete decongestive therapy (CDT).
The transfer is performed under general anesthesia. A section of tissue with lymph nodes and vessels is taken from the donor area and moved to the forearm or hand area, where it is connected to the blood supply. The patient is required to restrict limb movement in the area where the flap was transplanted while it heals.
The surgery requires microsurgical equipment, ultra-thin suture material, and microsurgical instruments.
During the healing period, it is important to restrict arm movement where the lymph nodes have been relocated. Hospitalization for this type of surgery lasts about five days. For the first two weeks, avoid lifting or straining your arm. Limit your range of motion. Do not wear compression garments or perform manual lymphatic drainage in the surgical area until completely healed. After eight weeks, you may resume normal physical activity.
