Carpal tunnel syndrome

One of the most common examples of compression-ischemic neuropathy is in which the median nerve is compressed in the narrow space between the bones and the transverse ligament of the wrist.
Compression neuropathies develop gradually, starting with a slight impairment of sensitivity, which over time can develop into severe muscle weakness and even a complete loss of movement in the affected limb. Thus, with the development of carpal tunnel syndrome, episodic sensitivity disorders first appear – numbness and tingling in the area of the thumb, index finger and middle finger. Then they are joined by burning pain, which can radiate to the forearm, and with the progression of the disease, pronounced muscle weakness makes habitual movements impossible. If treatment is not started on time, inflammation and swelling of the surrounding tissues worsen nerve compression, which can lead to persistent motor deficits. Carpal tunnel syndrome, like other tunnel neuropathies, is more common in people whose work involves repetitive hand movements – office workers, musicians, seamstresses, and drivers. However, the disease can occur without significant stress, for example, in systemic diseases (diabetes mellitus, rheumatoid arthritis), congenital abnormalities of tendon and bone structure, as well as after injuries and operations.

Reasons

Prolonged static loads

Repetitive movements of the hand or fingers

Injuries (fractures, dislocations, bruises, surgical procedures)

Congenital malformations of tendons and bones

Neoplasms in or near the nervous tissue

Systemic diseases (diabetes mellitus, rheumatoid arthritis, CNS infections)

Symptoms

Numbness, tingling, burning sensation in the innervation area of the affected nerve

Burning pain that increases with movement

Electric shock sensation, shooting along the nerve trunk

Muscle weakness, difficulty in hand movements

Local decrease in skin temperature due to impaired blood supply

Consequences

Compression neuropathy can lead to permanent nerve damage, which is manifested by irreversible muscle weakness and impaired coordination of movements. In addition to motor disorders, chronic nerve inflammation contributes to the occurrence of pain syndrome, which becomes permanent and difficult to treat.

Conservative treatment
Treatment begins by eliminating nerve compression and relieving inflammation. In the early stages, an effective method is the use of orthopedic fixators – bandages, orthoses and special splints that help relieve the affected area and prevent further damage to the nerve trunk. Drug therapy includes medications to relieve pain and edema. Physiotherapy methods help to reduce inflammation and accelerate the regeneration of nerve fibers. Therapeutic gymnastics and kinesitherapy help restore limb mobility.

Surgical treatment
The operation is aimed at eliminating mechanical compression of the nerve and restoring its functions. Depending on the cause of compression, various types of operations can be performed, including nerve release from scars and bone growths (neurolysis), and in severe cases– neurosis, in which a damaged nerve is connected to a healthy nerve bundle. After the operation, the patient is prescribed a course of physiotherapy and therapeutic gymnastics.
