Diabetic foot syndrome

A complication of diabetes mellitus, which is manifested by ulcers, infections and destruction of foot tissues.
Diabetic foot syndrome
Diabetic foot syndrome (DFS) develops against the background of a prolonged course of diabetes, when elevated blood sugar levels lead to damage to the vessels and nerves of the foot. This reduces sensitivity, impairs microcirculation and the ability of tissues to heal.

Reasons

Decompensated diabetes mellitus

Wearing tight shoes

Foot injuries (cuts, microtrauma)

Overweight

Fungal diseases of the feet

Circulatory disorders (atherosclerosis)

Neuropathy

Deformity of the bony skeleton of the foot and lower leg

Symptoms

Non-healing wounds and ulcers on the feet

Loss of sensitivity (neuropathic foot)

Pain in the feet (with ischemic form)

Swelling, redness and purulent discharge from wounds

Decrease in temperature and pallor of the skin of the foot

Cramps and tingling in the legs

The appearance of corns and blisters

Consequences

If left untreated, SDS can lead to the development of gangrene, which threatens amputation of the limb. Ulcers and necrosis can progress, especially against the background of ischemia, when tissues experience oxygen starvation. Wet gangrene, which is more common, can spread quickly, affecting not only the skin, but also muscles, tendons, and bones.

In addition, diabetic foot syndrome is associated with an increased risk of systemic infections, up to sepsis. Patients with SDS also face disability and a deterioration in their quality of life due to chronic pain and limited mobility.

Treatment

alt
Conservative treatment:
It is aimed at stabilizing blood sugar levels, improving blood circulation and controlling infection: Insulin therapy to normalize sugar levels Antibiotics to fight infection Angioprotectors and antiplatelet agents to improve microcirculation Anti-inflammatory drugs Unloading of the foot with the help of special shoes, orthoses, bandages Therapeutic compresses and bandages with antiseptics
alt
Surgical treatment:
It is performed in the presence of purulent complications, necrosis or critical ischemia: Primary surgical treatment of the wound (removal of necrotic tissues) Balloon angioplasty and stenting of the arteries of the lower extremities to restore blood flow Amputation (in case of irreversible tissue damage) of plastic and autodermoplasty to close ulcerative defects Reconstructive plastic surgery on the bone tissue of a deformed foot
