Cervicogenic headache (CGB)

Secondary headache caused by pathologies of the cervical spine, surrounding soft tissues or nervous structures. The pain starts in the neck and can spread to the back of the head, forehead, and temples.
CGB occurs due to a malfunction of the cervical spine, where the main nervous and vascular structures associated with the brain are located. Involvement of the trigeminal, large occipital nerve or degenerative changes in the II–III cervical vertebrae can cause pain. Most often, CGB manifests itself unilaterally, but with progression it can affect both sides. The pain is usually dull, constant and increases with movement of the neck or its prolonged fixed position (for example, when working at a computer). Spreading pain to the forehead, temples, and eyes can mimic migraine symptoms, making diagnosis more difficult.

Reasons

Degenerative changes in the cervical vertebrae.

Violation of blood flow in the vertebral arteries.

Neck strain or injury.

Poor posture and chronic muscle stress.

Symptoms

Constant, dull pain in the neck, occiput, extending to the head.

Increased pain when the neck is moved or held in a fixed position for a long time.

Pain in the forehead or temples.

Stiffness of the neck muscles.

Nausea, vomiting, dizziness.

Blurred vision.

Sensitivity to light or sound.

Numbness or weakness in the hands.

Consequences

Without timely treatment, CGB leads to constant discomfort and limited neck mobility, poor sleep, increased fatigue, and impaired coordination and limb weakness. In rare cases, with severe compression of blood vessels or nerves, persistent neurological disorders are possible.

Conservative treatment
CGB treatment usually begins with massage, physical therapy, manual therapy, and physical therapy, which help strengthen the neck muscles and improve its mobility. Medications are prescribed to reduce pain, and in some cases, injections of botulinum toxin to relieve muscle spasm. It is important to maintain proper posture, use orthopedic pillows and avoid staying in one position for a long time in order to reduce the strain on the cervical region.

Surgical treatment
Surgical intervention is extremely rarely required and is performed in the presence of persistent compression of nerve roots or vessels, which does not respond to conservative treatment. The main methods include decompression of nerves, elimination of bone growths or stabilization of vertebral segments.
