Face and neck wounds require a delicate approach, since the smallest injuries in this area can lead to impaired facial expression and speech, as well as to aesthetic defects. It is important to treat and restore the tissue quickly and as accurately as possible.
Primary surgical treatment (PST) helps prevent infection, minimize the risk of inflammatory complications and accelerate healing. Soft tissue plasty is also performed within a single procedure if necessary. This is relevant in case of skin and subcutaneous tissue loss. Current surgical techniques are designed to clean and close the wound, as well as to restore the integrity of anatomical structures: vessels, nerves, muscles, bones.
Preparation includes a visual examination and assessment of the depth and nature of the injury. In case of suspected vascular, bone, or nerve damage, additional ultrasound, X-ray, and CT scans are performed. The patient is also prescribed laboratory tests (complete blood count, coagulogram) and necessary specialist consultations (e.g., an ophthalmologist or neurosurgeon). For planned surgical interventions, standard preoperative screening is performed.
Depending on the extent of the procedure, patients undergo surgery under either local or general anesthesia. The surgeon excises the wound, removes contaminated and non-viable tissues, stops bleeding, and treats damaged structures (e.g., vessels, tendons, peripheral nerves) as needed. The wound is then sutured. If there is no severe contamination, suturing is performed immediately, which allows for healing by primary tension. In certain cases, skin and soft tissue plasty is performed simultaneously using rotational, advanced, or flap methods.
For primary surgical treatment, advanced microsurgical instruments, suture materials, antibacterial solutions, as well as aspiration and drainage systems are used.
The patient is prescribed a course of antibacterial therapy, dressings, and local antiseptic procedures. In the first few days, it is important to minimize facial expressions, physical activity, and exposure to sunlight. The sutures are removed after 5–10 days, depending on the area of damage and the healing process.
