Medial epicondylitis

A condition associated with overexertion of the flexor muscles of the hand and pronators of the forearm, which attach to the medial epicondyle of the humerus. It is one of the common causes of pain in the inner part of the elbow joint.
Medial epicondylitis, or "golfer's elbow," develops due to repetitive movements of hand flexion and forearm pronation, which is common in athletes involved in golf, baseball, swimming, or fencing. Increased stress causes microtrauma of the tendons and degeneration of the medial epicondyle of the humerus. The disease more often affects men aged 30 to 50 years, mainly the dominant arm. In half of the cases, ulnar nerve involvement is observed, which can cause pain in the forearm and hand, as well as muscle weakness. Despite the discomfort, the amount of movement in the elbow joint is usually preserved.

Predisposing factors

Reasons

Repeated flexion and pronation of the hand.

Excessive sports activities (golf, baseball, fencing, swimming).

Prolonged use of the dominant hand during physical activity.

Microtrauma of tendons in the area of the medial epicondyle.

Ulnar nerve involvement.

Symptoms

Pain in the area of the medial epicondyle of the shoulder.

Radiating pain in the forearm and hand.

Weakness of the forearm muscles.

Soreness on palpation of the inner surface of the elbow.

Discomfort when bending the hand or pronating the forearm.

Consequences

Without treatment, medial epicondylitis can lead to chronic pain and decreased forearm muscle strength. Permanent microtrauma of tendons increases the risk of tendon rupture. If the disease becomes chronic, irreversible changes in tendons and bones are possible.

Conservative treatment
It is recommended that the patient reduce the load on the affected muscles, temporarily stop or modify physical activity, avoiding movements that cause pain. In the acute stage, cold compresses and medications are used to relieve pain and inflammation. To stabilize and unload the tendons, an orthosis (brace) is used, which fixes the elbow joint and reduces the load on the medial epicondyle. Physiotherapy, massage, and manual therapy also help improve blood supply and tissue healing.

Surgical treatment
Surgical intervention is indicated in the absence of the effect of conservative treatment for 6-12 months or in the presence of significant degenerative changes. Surgery includes tendon release, removal of damaged tissues (debridement), tunneling of the medial epicondyle to improve blood supply, and tendon refixation. When the ulnar nerve is involved, it is revised to eliminate compression.
